The Sterling Value Added Orthotic Plan (arch support) enables members to receive orthotics at a price that is significantly
lower than "usual and customary" retail costs. An orthotic device is a specially formed supportive device to be worn in your
shoes. It acts as an adaptor between your foot and the floor or ground that you walk on. Custom orthotics are made from molds
of your feet an adapt to the specific contours of your foot.

space at the upper right hand corner of the form. Upon receipt of the kit, fabrication of the appliance and delivery takes
about two weeks. Members have a 30-day refund policy if the orthotic appliance needs to be returned for any reason. (Shipping
and handling fee is not refundable.)
