Be Sure To.....

a) Completely fill out the information on all pages of the Enrollment Application Form and initial statements where indicated.

b) On page 4, be sure to sign and date the Application. You'll need your Medicare Card to fill out the bottom portion on Page 1.
(Do not send in your Medicare Card, simply copy the information on the card to this page 1 of the Application.)

Enclose the following:

a) Completed four-page Enrollment Application Form.

b) First month's premium in the form of a check or money order made payable to Sterling.

c) Mail To:

Once your completed Application and premium are received and processed, you will receive a Welcome Packet containing a wealth of information including a letter confirming your enrollment and its effective date as well as your Sterling Option IV^SM Identification Card.

A Medicare Advantage Private Fee-for-Service plan works differently than a Medicare Supplement plan. Your doctor or hospital must agree to accept the plan's terms and conditions prior to providing healthcare services to you, with exception of emergencies. If your doctor or hospital does not agree to accept our payment terms and conditions, they may not provide healthcare services to you, except in emergencies. Providers can find the plan's terms and conditions on our website at www.sterlingplans.com (or the direct link to the provider terms and conditions of payment can be found online at: https://secure.sterlingplans.com/providers/pdf/option4tandc.pdf).
